If it is about a Cohort Radar payment, include the email address you paid with — that is the only thing a payment can be found by. If you want a refund within 30 days of paying, you may not need this page at all: open your results page in the browser you bought it from and there is a refund button on it, which reverses the charge without asking anyone.
